The article summarizes some of the side effects of antipsychotics that are not common in everyday clinical practice, although the patients were treated by commonly used antipsychotics. The information from literature is supplemented by cases of the patients treated by us. We focused on thromboses and anticholinergic deliria, but some other side effects are mentioned as well. The causes, circumstances of initiation and clinical management of these side effects are disscused.

Sleep disorders as a symptom can accompany a variety of conditions, whether mental or physical. They do not always mean illness and at a certain stage of life almost every person has struggled with it. Sleep disturbances, both in terms of shortening, prolonging or changing quality, are a common symptom of mood disorders. The fact that chronobiological treatment leads to mood swings also adds to the interrelationship. Insomnia is also considered a significant risk factor for the development or recurrence of depression, as well as being examined as a predictor of response to treatment. The following article provides a brief overview of the issue of sleep...

Mixed anxiety and depressive disorder is a diagnostic category in the 10th and 11th editions of the International Classification of Diseases (ICD). It can be made when a patient shows co-occurring anxiety and depressive symptoms causing significant distress but not expressed in such severity that another anxiety or affective disorder can be diagnosed. Sometimes the existence of this disorder is questioned because of the low severity of symptoms, the difficulty of delineating it from similar conditions and the low diagnostic reliability. For example, it does not appear in the most recent fifth edition of the Diagnostic and Statistical...

Psychogenic nonepileptic seizures (PNES) are the second most common functional neurological disorder after movement disorders. They are paroxysmal pathological behavioral manifestations or experiences, which clinically resemble epileptic seizures or syncope. However, they do not have the same pathophysiological cause (i.e. electrophysiological correlate typical of seizures or hypoperfusion of the brain typical of syncope). Although it is a functional neurological disorder with characteristic features, its diagnose is often difficult. It usually lasts several years from the first attack to making a correct diagnosis, during which patients are inadequately...

Prevalence of cognitive impairment in multiple sclerosis (MS) is reported to be in range between 35% and 65%. Cognitive impairment in MS has usually a mild form with information processing speed and episodic memory being the most affected cognitive domains. Annual neuropsychological screening of people with MS can provide clinicians with useful data on disease activity and progression; even before the threshold for cognitive impairment is reached. Neuropsychological test batteries BICAMS, MACFIMS or the SDMT test are recommended as the tools of choice for neuropsychological monitoring of MS. The change in cognitive outcomes can be evaluated by Reliable...

The case study describes a case of involuntary treatment for an unmotivated 37-year-old patient with a diagnosis of alcohol dependence, who was ordered by the court to undergo institutional protective anti-alcohol treatment, which was supposed to take place while serving a prison sentence. The patient began inpatient protective treatment in a specialized section of the prison. Despite all the efforts of the multidisciplinary team, the patient did not complete the treatment. After many months, the court canceled the protective treatment due to its ineffectiveness. The patient did not want to work on himself, completely denied his diagnosed alcohol addiction,...

Olanzapine is an antipsychotic medication. Its indications include psychotic disorders and manic phases in bipolar affective disorder. The mechanism of action is multi-receptor, which results in its effects not only on the positive symptomatology of psychotic diseases. The dispersible form of olanzapine is bioequivalent to the classic tablet form. The efficacy and safety of this variant make the dispersible form a suitable choice for various psychiatric conditions.

Psychosomatic medicine deals with the interaction between psychosocial and biological factors participating in the origin and development of psychosomatic diseases and disorders, as well as with their course, treatment and prevention. It does not include those theoretical approaches that deny the possibility of the psychological influence on emergence and development of diseases, on the other hand, nor concepts that do not pay any attention to the physical aspects of the illness. This defines the boundaries of psychosomatic medicine, scientifically based and developed medical speciality, in which the Psychosomatic Clinic in Prague is engaged. In the...
