Psychiatrists attitude to antipsychotic depot treatment

At first history and current use of treatment with depot antipsychotics (AP) is summarized. Nowadays also the 2nd generation APs in depot formulation are available. With long-term treatment the depot APs are better in the relapse prevention in comparison with oral APs. Because psychiatrists diagnose and choose the treatment, their attitudes to different formulations of treatment are important. Psychiatrists seem to use depots in a conservative way and mostly introduce them after several episodes and for reasons of nonadherence. When you deploy pharmacotherapy in children you need to consider the following aspects: development, maturity, society and communication. The article discussed only pharmacotherapy medications although of course a treatment would include a combination of psychotherapy, pharmacotherapy and sociotherapy. Marginally there are mentioned findings in well-defined diagnostic units by imaging methods and practices supportive of some therapeutic procedures „off-label“.

The pharmacological treatment in old age has its specifics and the polypharmacy belongs among the most common problematic situations in gerontopsychiatric patients. Safety monitoring of the treatment with risperidone in children and adolescents.

Risperidone is an atypic antipsychotic frequently used in children. Risperidone was found to be beneficial and safe in low doses in the treatment of conduct disorders in children and adolescents. Studies in children report significant weight gain and increase in BMI, extrapyramidal side effects and variable elevation in prolactin levels, especially in long term risperidone use. Guidelines of Canadian Alliance for Monitoring Effectiveness and Safety of Antipsychotics in Children (CAMESA) are specifically focused on metabolic and neurological side effects, and how they should be monitored. A psychotherapeutic treatment focalised on a multimodal approach of using imaginative methods of visual art and verbal input into the content and form of created files has been apply within a treatment of a patient suffering from spinal pain and headachesince his childhood. We worked through expression and reception. The following case study introduced causal analytical work using symbolic form and symbolic confrontation with pain.

Depressive disorders occur in about 40% of patients with dementia. They worsen cognitive dysfunction and the level of functioning, leading to behavioral disorders. They reduce the quality of life of patients and their carers and increase the risk of institutionalization. They often remain undiagnosed or are treated wrongly. Thus, targeted screening and timely and correct treatment of these diseases are necessary.

The article focuses on the symptoms and on controlling mental problems associated with a break-up. Common phases of so called psychobreak-up and their manifestations in emotional, cognitive, physical and behavioral dimension are descripted. Mostly cognitive behavioral methods for coping with related problems are recommended. In the end of the article a case study describing complicated progress of psychobreak-up and corresponding psychotherapy is presented.
