The treatment of sleep disorders has always been a complex process that includes behavioral, neurophysiologic, psychological, chronobiological and, not in the least, pharmacological factors (1). Negative influence of different pharmaceuticals use to be very often neglected or underrated. In reverse, an extent of positive treatment possibilities of pharmacotherapy has been perceived inconsistently very often at present. The simple classification of pharmaceuticals for insomnia into sedatives/hypnotics and stimulants respectively, can’t stand up in the context of nowadays knowledge about neurophysiology and chronobiology of sleep process.

Multiple sclerosis (MS) presents the most frequent demyelinating disease of the central nervous system (CNS) with autoimmune ethiology. Immune system plays a key role in MS pathogenesis and any CNS area can be affected by its inflammatory activity with various neurological and psychiatric symptoms. Antidepressants in the treatment of bipolar depression

The use of antidepressants in the treatment of bipolar disorder remains controversial. Despite the frequent use in clinical practice, the fundamental questions regarding the efficacy and safety of antidepressants in both acute and long-term treatment of bipolar disorder have not yet been satisfactorily answered. The article offers a current view of the place of antidepressants in the treatment of bipolar depression, including general recommendations for clinical practice.

Psychotherapy in psychoses

An article deals with a development of psychotherapy in psychoses, with the most important areas of problems that the psychotherapy in psychoses targets (symptoms resistant to drugs, cognitive deficits, a failure in social roles or social skills, problems in relationships) and with an effectiveness of psychotherapy. Individual, group psychotherapy, family therapy, cognitive rehabilitation and psychoeducation are discussed separately.

Detoxification of children and adolescents addicted to non-alcoholic addictive substances

Although in 2006 positive tendencies appeared in the area of using illegal drugs in the Czech Republic, there is still a group of children and adolescents, in whom the need for detoxification occurs. The problematic drugs are most often volatile substances and cannabis drugs in children, pervitin and heroin in the adolescent period. Motivation and family therapies are an important part of detoxification which is mostly done during hospitalization. The views of developmental psychology must not be neglected while choosing a therapeutic approach. The bases of detoxification methods and treatment in the most often used drugs are mentioned in the article.

Psychotic symptoms in methamphetamine dependent subjects

This is a review article on methamphetamine-induced psychosis. About 20 000 subjects in the Czech Republic abuse methamphetamine (MAMP). The prevalence of psychotic states in this subpopulation is eleven times more frequent as compared to general population. Genetic factors, premorbid personality and methamphetamine itself with its neurotoxic and neuropathophysiological effects are important in etiopathogenesis of MAMP psychosis. The clinical picture with paranoid-hallucinatory symptoms resembles an acute episode of schizophrenia, negative or affective signs may also be present. A case report is presented of a 23-year-old woman who presented to my psychiatric practice with depressive mood in 2007. She then had a boyfriend and was working in a garden centre. In a short time interval, she underwent repeated inpatient treatment during which a wide number of psychiatric drugs were administered.
