Odvykací stav s deliriem po odnětí zolpidemu

V kazuistice jsem sledoval průběh hospitalizace pacienta závislého na zolpidemu. Zpočátku se u něj projevil odvykací stav s tremorem, úzkostí, obavami, následně se objevily přechodné orofaciální dyskinézy a poté následoval několikadenní delirantní stav ze snížení hladiny preparátu. Po odeznění deliria a krátkém zlepšení stavu došlo k rozvoji paranoidní sympto­matiky dobře reagující na risperidon a carbamazepin. Na uvedeném případě je možné vysledovat přítomnost všech známek závislosti, včetně přítomnosti následného odvykacího stavu s deliriem ze snížení hladiny. Lze tedy hovořit o riziku vzniku závilosti na zolpidemu, přestože je v širokém povědomí odborné i laické veřejnosti dosud vnímán jako bezpečné nenávykové hypnotikum.
