Pokrok v této oblasti přispěl k zavedení tzv. polygenního rizikového skóre (polygenic (risk) score, PGS), které měří genetické riziko na základě běžných genetických variant identifikovaných pomocí celogenomových asociačních studií. Je vypočítáváno jako vážená suma rizikových alel u konkrétního jedince. Z výzkumu vyplývá, že psychické poruchy jsou vysoce polygenní a geneticky heterogenní (29). V recentně publikované genetické studii TRD se heritabilita pohybovala v rozmezí 2–4,2 %, a bylo pozorováno signifikantní překrývání genetických variant s kognicí, hyperaktivní poruchou, schizofrenií, tendencí k abúzu návykových látek a BMI (body mass index) (30). Sdílená biologie má potenciál vývoje nových farmakologických přístupů. Závěr V celé medicíně se snažíme o individualizovanou léčbu. V psychiatrii vycházíme při volbě léčby ze znalosti současných možností psychofarmakoterapie, pečlivé anamnézy a zhodnocení psychického stavu včetně klinického obrazu. Zatím nemáme v běžné klinické praxi markery, které by nám v rozhodovacím procesu pomohly. Proto bychom od počátku měli usilovat o optimalizaci stávající léčby (využití terapeutického monitorování léku, farmakogenetického testování metabolizujících enzymů) a včasné stanovení farmakorezistence.
